Transition assessment timing?

In late high school, around age 18.

Only after high school graduation.

In early adolescence, around 14–16.

Focusing on transition early—around ages 14–16—gives students time to explore interests, gather relevant data, and start building a plan for life after school. This window aligns with requirements that transition planning should be underway no later than mid-te adolescence, so the team has several years to complete assessments, set concrete postsecondary goals, and arrange supports like coursework, work-based learning, and community services before exiting school. Beginning too late, such as after graduation, leaves little to no opportunity to implement the plan or practice independent skills. Starting too early, in elementary years, wouldn’t yet reflect the more complex options students face later.
